Home and Community Care & Caregiver Support

The goal of the Home Care, Community and Caregiver Support Services Team of the SHIH is to work with existing service providers to improve these services on a community basis. We are discussing options with service providers, reviewing the literature and learning as much as possible about the existing system in order to develop a community-based innovation. In the course of educating ourselves and the community, we developed an online guide and held a hybrid learning session.

The Guide to Healthy Aging in the Community

We developed an online resource Guide for Aging in Our Community, currently hosted by the OSCA website.

We received a grant from New Horizons for Seniors to develop a printed brochure to publicize the online guide and to serve as a community feedback instrument to permit self-identification of people needing primary care, to recruit volunteers for testing of new seniors’ technology and explore ways to support family and friend care provider.

After the session, the Home Care and Community Support Working Group developed clear concise guidance on how to contact Ontario Health atHome. As well, we have a short summary on what services the City of Ottawa, through Ottawa Public Health, provides to older residents.
